Hey Sid...
I've been running USR 28.8's since the day after they came out :-)
They are not very "phoneline responsive", in other words, if you have poor phonelines, you WILL notice a difference from day to day with this modem, especially in the winter when the weather gets bad.
I have no idea how they work with the Keyspan SX-4 card, sorry.
Here are my init strings, they seem to work for me :-)
